What Is the EXERT Trial?
The EXERT trial, led by UC San Diego and Wake Forest University, is the largest and longest study of its kind, exploring how physical activity can impact people living with mild cognitive impairment (MCI)—a common early stage of memory loss that can lead to Alzheimer’s disease.
In this trial, over 300 older adults aged 65 to 89 with MCI were enrolled and randomly assigned to one of two exercise programs:
- Moderate-intensity aerobic exercise, such as brisk walking, cycling or swimming.
- Low-intensity stretching and balance routines similar to yoga or tai chi.
The participants exercised four times a week over a 12-month period, all while being supported in local YMCA facilities—creating a real-world model of what’s possible in community-based care settings.
The results were encouraging. Both groups maintained cognitive function over 12 months, defying expectations of continued decline in individuals with MCI. Alos, brain scans showed better preservation of brain volume, especially in areas related to memory and executive function.
The social and structured environment of group exercise also added emotional and psychological benefits, improving mood and reducing isolation. In short, regular movement—even at low intensity—can help protect memory and slow cognitive decline.
